True to size has suited me very well with 634S.

Go to the how we measure page:

read that, then compare the measurements as we suggest on the sizing tab, then make your mind up. I can't choose for you.

Some iPhone quickies.
I measured my Roy's after wearing them for a couple weeks so the waistband was stretched out. They were a tagged 33, but the waist was stretched to a 34.25. I bought a 33 in the 634s. If I had bought the 34, they might have been a bit baggy through the thigh in my opinion. The 33 was perfect, Tight in the waistband the first couple days, but feeling really good now, been about three days. You don't want to go to big in my opinion, remember your "goal" is to wear the shit out of them before you wash them. you might wear them 200 times before you wash them. That shit will stretch out. Do what Giles said and then go grab your favorite pair of pants and measure them, preferably after you have worn them a bit, And measure your thigh on your favorite pair to compare to the 634s chart!!! Very important!! If you don't think the thigh will work on a pair that is an inch smaller in the waist than your " true size", then! size up to your true size in my opinion

Like Giles said, measure your favorite pair of jeans, following the guide. If you have a pair of jeans that fits like crap, measure them, too. See where the differences are and how much the differences are. Figure out your appropriate size using the 634S size charts.
It's not as intimidating as it might seem initially. The 21 oz denim will stretch some, but that doesn't mean you should buy jeans that are too small and hope they'll fit eventually. You want them to feel great on the first wear, 100th wear, the 500th wear, etc.
Oh, and don't caught up in the crazy urban myth care hype- just wash them when they get dirty. An ordinary washing machine and ordinary detergent are fine. There aren't any secrets. In the end, it's just a pair of jeans.

besides inseam measurement (since i am incredibly tall I am always f'd when it comes to finding proper measurements) I normally am most concerned with the fit in the thigh in oppositioin to the waist and personally feel like that number is generally much less flexible than the waist. Generally speaking a waist measurement can be stretched quite a bit if needed, or even better fits great from the start and will stretch out and shrink back to size when washed, but the thigh measurement is much less forgiving.
Thigh= less stretch and belts dont fix if they are a hair to big for your preferencehope that makes sense
-
Personally, I concentrate on the thigh measurement. Inseam isn't that important, because I'm pretty ordinary height-wise, unlike you, tatman. However, my thighs are on the thick side, so I can't wear a lot of the slimmer styles. I had a bad experience with a pair out Samurai jeans that I've sworn never to repeat.

Might be your goal, but wearing jeans 200 times/days without washing isn't everybody's goal.
Wearing jeans without washig them is a hype like Chris stated…

I love how since everyone watched the kiya " the denim icon" denim myths, how much their opinion has changed. So you're telling me, had Max power washed his over dyed 634 everytime they got a little dirty that they would still have such sick fades.
Chris, skimmed through your pics, but never once saw a good fade pic. I'll wear my pants without washing them and you guys can wash yours to much and we will see whos looks better.
-
We talk about a Iron Heart Product. They all look good, washed or not. Do it your way, but please don´t see fades as a competition or something like that. You should feel good in your clothes, if you feel good in smelly and dirty jeans, that´s cool for you. Like I said before, I don´t wash my jeans every week, but I wash them when they are dirty or smell very bad.

Yep - and I've never had any of my jeans smell bad… I just wash 'm cause they do get dirty after a couple of months... And because I feel like I "should" do it... That's why I'm not doing it for a year... I will need a objective jury to come sniff my jeans in June 2014 to see if I'm right about them not smelling bad...
-
Haven't washed my 634s yet. They are so thick an pliant that getting creases to set in sharply is a challenge. So I will wait a little on them unless they get funky or dirty.

And this is only for jeans!!!!! You wanna wear your shirts for months on end that's your business, Just don't stand next to me:) I can't go much longer than a couple weeks without washing a shirt. I wore my wabash western for about two weeks, had to wash it Sunday after it had umpteen beers spilled on it Saturday night!!
