I went with the advice to size down and bought S for my 38-39" chest size. I can wear it over a t-shirt (loose fit) or tee plus IHTL-1213 Thermal Waffle Henley for a more snug fit.
For those who are not sure about their size in this, remember the arm opening does not actually come up close under your armpit - it is a couple of inches below mine.
The cotton duck back suits it very well and is extremely supple. The denim front and sides feel thicker and are stiffer than the back, but the 12 oz denim feels like the right choice for this type of vest/waistcoat - not too stiff, but still rugged enough.
To attempt to shrink it a little, I washed it inside out on a 40 degree Celsius (104°F) cottons cycle with Persil colour powder (for those who care about these kinds of details). Then I put it on a hanger on a radiator. It dried very quickly and there were no creases or apparent colour loss; it still looked brand new.
After wearing it for several hours over a sweatshirt I measured it and realised my attempt to shrink it had failed, or it had stretched back out with wear.

Considering picking one of these up… been awhile
I typically wear a large in IH Shirts and Jackets and a 42 in FH, what size should I go for on this?

@Giles I'm doing some research as I'm planning to buy this or the double indigo. I've measured the two vests I own the IH way and most measurements are similar to the L in the IHV-02-IND, with the exception of the chest which is similar to the XL. My question is this, have you ever re-measured your IH vests after wear? I guess there must be some stretch horizontally across the back and chest?
